Heparin Cofaktor II (HC II) ist ein spezifischer Thrombininhibitor und hemmt im Unterschied zu Antithrombin Faktor Xa nicht. HC II kann die Antithrombin-Bestimmung über ein Thrombin-basiertes Testsystem beeinflussen. Testmethoden, bei denen Faktor Xa eingesetzt wird, erfassen ausschließlich die Antithrombin-Aktivität. Die Komplexbildung zwischen HC II und Thrombin wird durch Heparin und Dermatansulfat erheblich beschleunigt. In vivo wird HC II inhibtorisch erst dann wirksam, wenn der Antithrombin-Spiegel auf ca. 30% abgesunken ist. Größere Bedeutung scheint HC II als Thrombininhibitor nach Verletzungen im extravaskulären Raum zu besitzen. Darauf deutet auch die Verstärkung der Hemmung durch Dermatansulfat hin, das nur dort vorkommt.
Klinische Bedeutung
Der hereditäre Heparin Cofaktor II-Mangel ist mit einer Thromboseneigung verbunden. Ein erworbener Mangel wird bei Lebererkrankungen, bei Diabetes mellitus und bei Verbrauchskoagulopathien (DIC) beobachtet. Erhöhte Heparin Cofaktor II-Spiegel sind nach Einnahme oraler Kontrazeptiva, bei Schwangerschaft und mit zunehmendem Alter feststellbar.
