The bovine Kunitz-type inhibitor aprotinin influences the intrinsic system of coagulation and fibrinolysis by inhibiting kallikrein. Apart from the main effect of aprotinin on kallikrein, further serine proteases such as trypsin, chymotrypsin and plasmin are inhibited. The bleeding complication reduction of aprotinin is to some extent also likely due to its inhibition of urokinase, APC and the factor VIIa-TF-complex.
Clinical significance
Aprotinin is applied in a dosage of 200 KIE/ml plasma especially in thorax surgery, to reduce intra- and post-operative blood loss and, therewith, transfusions. Its exact mechanism of action is as yet unknown. Apart from the retardation of fibrinolysis through the inhibition of kallikrein, an inhibition of platelet activation is being discussed.
