Der Vitamin K-abhängige Faktor X steht an zentraler Stelle des Gerinnungssystems. Er wird durch Faktoren des intrinsischen Systems (Tenase-Komplex aus Faktor IXa, VIIIa, Phospholipiden und Calciumionen) und durch den Tissue Factor/Faktor VIIa-Komplex des extrinsischen Systems aktiviert. Faktor Xa als Serinprotease bildet mit Faktor Va, Phospholipiden und Calciumionen den Prothrombinase-Komplex, an dem die Aktivierung des Prothrombins zu Thrombin stattfindet. Faktor Va fungiert als Cofaktor, der die Aktivierung um das 1000-fache verstärkt.
Klinische Bedeutung
Die hereditäre Form eines Faktor X-Mangels wird autosomal rezessiv vererbt, ist jedoch selten. Ein erworbener Mangel tritt wie bei allen Vitamin K-abhängigen Faktoren bei Vitamin K-Mangel oder einer Coumarintherapie auf. Zu erniedrigten Faktor X-Spiegeln führen Lebererkrankungen, eine L-Asparaginasetherapie sowie eine Verbrauchskoagulopathie. Die Symptomatik eines Faktor X-Mangels reicht von Gelenk- und Schleimhautblutungen sowie postoperativen Nachblutungen bis hin zu Gehirnblutungen. Erhöhte Faktor X-Spiegel stellt man als Reaktion auf Vitamin K-Gabe und bei Hyperlipidämien fest.
Indikation
- Diagnose eines bislang unbekannten, hereditären Blutungsleidens
- Ursache eines pathologischen Quick-Wertes
- INR-Einstellung bei Patienten mit pathologischem Quick-Wert aufgrund von Hemmkörpern
- Kontrolle des Plasmaspiegels während einer Antikoagulanzientherapie mit Heparin oder Coumrin-Derivaten (während der Übergangsphase) sowie bei einer fibrinolytischen Therapie
- Verlaufskontrolle zur Differenzialdiagnostik zwischen Synthesestörung und erhöhtem Verbrauch
- Kontrolle des Plasmaspiegels während einer Antikoagulanzientherapie mit Thrombininhibitoren
- Kontrolle einer Antikoagulanzientherapie bei Patienten mit einem Antiphospholipid-Antikörper-Syndrom
