Dabigatran Level Determination
Calibrators and controls now available
Dabigatran (Pradaxa®) is a direct thrombin inhibitor. It is the active component of the oral anticoagulant prodrug Dabigatran Etexilate.
Dabigatran Etexilate is a low-molecular-weight prodrug that exhibits no pharmacological activity. Dabigatran Etexilate is rapidly resorbed after oral application and, via esterase-catalyzed hydrolysis in plasma and in the liver, converted to its active form Dabigatran. Dabigatran is a highly effective, competitive, reversible and direct thrombin inhibitor. The inhibition of thrombin prevents the thrombus formation, as thrombin causes the conversion of fibrinogen to fibrin in the coagulation cascade. Furthermore, Dabigatran inhibits both free and fibrin bound thrombin as well as the thrombin induced platelet aggregation.
In case of compliance, testing or suspicion of over dosage a quantitative activity determination might be necessary. That requires a suitable routine and automated test system.
There is now with TECHNOCLOT® DTI a diluted thrombin time test available that by using suitable calibrators and controls allows reliable level determination of Dabigatran.
