Der Kunitztyp-Inhibitor Aprotinin (Rind) beeinflusst über die Hemmung des Kallikreins das intrinsische System der Gerinnung und die Fibrinolyse. Neben der Hauptwirkung des Aprotinins auf das Kallikrein werden weitere Serinproteasen wie Trypsin, Chymotrypsin und Plasmin gehemmt. Die Reduktion von Blutungskomplikationen wird zum Teil auch der Hemmung von Urokinase, APC und dem Faktor VIIa/Tissue Factor-Komplex durch Aprotinin zugeschrieben.
Klinische Bedeutung
Aprotinin wird in einer Dosierung von 200 KIE/ml Plasma vor allem bei thoraxchirurgischen Eingriffen eingesetzt, um intra- und postoperative Blutverluste und damit Transfusionen zu verringern. Der genaue Mechanismus ist noch unbekannt. Neben der Verlangsamung der Fibrinolyse durch die Hemmung des Kallikreins wird eine Verhinderung der Thrombozyten-Aktivierung durch Aprotinin diskutiert. Die Reduzierung von Blutungskomplikationen durch Aprotinin scheint z.T. auch durch die Inhibierung von Urokinase, APC und den Faktor VIIa-TF-Komplex bedingt zu sein.
