Thrombingenerierung und Emicizumab

Zusätzliche Erkenntnisse aus der Bestimmung des Thrombingenerierungspotentials

Thrombingenerierungs Assays (TGA) basieren auf der Bestimmung der Thrombinbildung mittels eines fluorogenen Substrates nach einer Aktivierung durch Tissue Factor und Phospholipide. Diese Teste können zur Überprüfung von Haemophilen während einer Inhibitor-Bypass-Therapie verwendet werden.

Im Falle eines Blutungsereignisses unter Emicizumab-Therapie benötigen Patienten mit Inhibitor zusätzlich die Gabe von BPAs (Bypassing Agents).

Die zusätzliche Gabe von APPC (activated prothrombin complex concentrate) kann problematisch sein. Ein vielversprechender Ansatz hin zu einer maßgeschneiderten Therapie könnte die Bestimmung des Thrombingenerierungspotentials sein.
